The Victorian Government wants to hear from people with a disability.

Are you a person with a disability who is over 18 years old? The Victorian Government wants to know about your experiences, in your life and your community, since the State Disability Plan 2013 – 2016 commenced on 1 January 2013.

You can shape the future of the plan and help the Victorian Government improve services and opportunities for people with a disability by taking the Victorian State Disability Plan survey.

The survey will be open until 6 November 2015.

Your answers will be anonymous and kept private. They will only be used for this research. The survey will be held every six months and the answers from each survey will be compared to see if anything has changed.

Work has now begun on the Victorian Government's next State Disability Plan for 2017 – 2020. Feedback from this survey will help to shape the next plan.

If you have any difficulties completing the survey or would like to take the survey in a different format (by phone or paper copy) please email us at statedisabilityplan@dhhs.vic.gov.au or call 1300 880 043.
